Job Description:

The Safe Care Home Visitor will provide an evidenced-based, intensive intervention to families with children in assigned county.


  • Typical hours include Monday-Friday 9:00am - 5:00pm with ability to work evenings and weekends as needed
  • Rotating on call responsibilities required


  • Carry a full caseload of 12-15 families as assigned
  • Actively participates as part of a team of service providers and others with focus on well coordinated services for family members
  • Performs the basic accountability tasks related to the job which include compiling and maintaining all necessary records, forms and reports
  • Attend required trainings as needed and adhere to the Safe Care Model
  • Responsible for maintaining the privacy and confidentiality of Protected Health Information of service recipients in any form (eg., computer access, conversation, document) during and surviving the termination, expiration or cancellation of employment/association with CHC
  • Assists administrative staff in collection of information for program effectiveness
  • Responds in a positive and respectful manner to diversity, among the agency’s service population and staff



Experience and Skills:

  • Bachelor’s degree in related field and a minimum of 1-year experience in family and children’s services with experience working from a strengths perspective with families
  • Ability and willingness to work with families in an 'in-home' context
  • Ability to internalize and work within guidelines of the Safe Care Model
  • Knowledge of family systems theory and home-based interventions
  • Ability to engage families who may not want the service or who are mandated
  • Ability to have flexibility in hours worked both routinely, so as to meet children and families when they are available, and occasionally, in response to crises which may occur at any hour of any day
  • Valid NYS Driver's License
  • Vehicle required: Must be safe and dependable
  • Ability to travel to other agency locations as required

  • Walking up and down stairs inside/outside
  • Must have ability to type and hand-write documentation and maintain proper files
