Job Description:

SUMMARY OF RESPONSIBILITIES: Assist with the provision of primary health care services to children and adolescents in our Out of Home Care programs. Working collaboratively with the Director of CNY Health Services in an office setting, our Registered Nurses (RN) will coordinate reproductive health care services for adolescents, and provide nursing oversight for our services for agency programs.

FUNCTIONS:

·         Health history/screening for substance abuse, development, mental health, physical body checks for abuse

·         Conducting assessments, including:

o    Basic first aid, phone and in person triage

o    Intake assessments including Height/Weight/Vital signs, and screening for abuse/neglect

o    Visual screen for infection/condition/infestation

o    Assessment and treatment plan for acute illness/injuries

o    HIV risk assessments and screening when indicated

o    Assessment and treatment plan development for management of chronic health conditions (e.g., diabetic care, asthma care), including training of children in foster care and foster parents

·         First Aid/BLS and minor wound care

·         Administer immunizations

·         Urine specimen collection

·         Venipuncture

·         Medication administration and management including reconciliation

·         Assistance with self-administration of medications

·         Training and health education including reproductive health

·         Support and management of the emotional/psychiatric needs of children

·         Assessment post AWOL (may combine with nursing assessment)

·         Establishing treatment goals for children, utilizing historical and current available information in collaboration with the multidisciplinary team

·         Confirming adequate nursing staff coverage hours

·         Maintaining medical supplies and equipment

·         Practicing proper infection control

·         Conducting follow-up after medical appointments, urgent/emergency care, or hospitalization (i.e., Providing and coordinating nursing care)

·         Providing/Assisting with home visits, educating caregivers and staff, and monitoring healthcare needs

·         Assisting with community provider visits

·         Provide professional nursing oversight, including certification for administration of medications, to individuals in designated programs

·         Responsible for maintaining the privacy and confidentiality of Protected Health Information of service recipients in any form (e.g., computer access, conversations, documentation) during and surviving the termination, expiration or cancellation of employment/association with Cayuga Centers

·         Responsible for complying with rules and regulations governing access to Protected Health Information under HIPPA. Access to Protected Health Information is limitied to the extent required to perform responsibilities effectively and efficiently.  No access will be permitted unnecessarily. These essential job functions dictate the level of access, use, and disclosure of confidential information.

·         Perform other duties as requested by program supervisors and agency leadership.

 





Experience and Skills:

  • Valid unrestricted NYS RN license required
  • Associate Degree in Nursing required, Bachelor of Science in Nursing preferred
  • Strong communication and organizational skills
  • Ability to work effectively in a team context
  • Demonstrate ability to perform duties with minimal supervision and to exercise greater judgment
  • Ability to clearly communicate both in writing and verbally
  • Ability to work effectively and patiently with others in difficult and/or stressful situations
  • Ability to handle confidential material in a responsible manner
  • Valid NYS Driver’s License required
  • Vehicle required: Must be safe and dependable
  • Ability to travel to all agency locations as required

PHYSICAL REQUIREMENTS:

  • Word processing: Including responding to e-mails, composing letters, completing computerized charting
  • Documentation and transcribing by hand when needed, etc.: This may involve sitting at a computer and/or documenting by hand for several hours
  • Completing paperwork -Such as copying, stapling and filing as needed
  • Must be able to lift up to 20 pounds
  • Walk up/down stairs as needed

HOURS PER WEEK:  Exempt

  • Regular hours entail forty hours per week
  • 8:30am - 4:30pm Mon-Thurs and 11:00am – 7:00pm Fri
  • Participation in on call rotation for nursing triage during off hours required
