Job Description:

Under the Supervision of the Manager of Care Management Services, the Intake Care Manager will track referrals and conduct initial outreach, engagement, and enrollment services to children presumed to meet eligibility requirements of New York State’s Children’s Health Home program. This Case Manager will make initial connections with referral sources to gather assessments and enter documentation, engage the youth and family in services, identify initial areas requiring support services, and complete the CANS-NY assessment. Following assessment, the Intake Care Manager will work with the youth and family to set the initial Crisis and Emergency Plans in conjunction with the ongoing Care Manager. This Care Manager will serve the Finger Lakes and Central Regions of the agency, and at times will cover the Western Region as the need arises.

FUNCTIONS:

  • Review and track all referrals made to the Care Management Program for the Finger Lakes, and Central Regions

  • Ensure that all youth referred meet eligibility criteria

  • Connect with referral sources to gather all information required for program enrollment

  • Complete all tracking forms and update program rosters and billing sheets

  • conduct and document outreach activities, as required by the agency and contracting Health Homes

  • Engage youth and families in initial program activities and document all engagement efforts as required by the agency and contracting Health Homes

  • Conduct and document all enrollment activities, as required by the agency and contracting Health Homes

  • Provide education to family and children concerning the nature and desired effects of services, along with information concerning conditions being treated

  • Enter cases into the MAPP, Netsmart, and other electronic portals and electronic medical records

  • Conduct the initial CANS-NY assessment to establish acuity

  • Complete the initial Emergency/Crisis plans

  • Facilitate transfer meetings with the Care Manager who will oversee ongoing Care Management Service

Experience and Skills:

  • Master’s degree in Human Services related field with a minimum of one year of experience with an in-home serve model and/or case management services; or
  • Bachelor’s degree in Human Services related field or in Education with a minimum of two years of experience with an in-home serve model and/or case management services; or
  • Registered Nurse with a minimum of two years of experience
  • Experience in Care Management and/or providing direct case management services to youth and families preferred
  • Experience in serving children and families within the child welfare system, those with developmental disabilities, behavioral health symptoms, primary health care concerns or social services involvement is required
  • Experience in engaging youth in services who may exhibit challenging behaviors in various settings (i.e. home, foster home, school, work)

  • Experience in coordinating and participating in family-team settings

  • Experience in the delivery of family-driven, youth-guided services

  • Ability to support and provide feedback to youth and their families as they engage in recommended treatment programs

  • Ability to understand child development and programs that provide treatment and support youth who have experienced trauma and co-occurring mental health problems

  • Ability to respond to youth and families in crisis, utilizing de-escalation strategies, and provide home based stabilization on a rotating on-call basis

  • Valid NYS Driver’s License required

  • Safe and dependable vehicle required

 

Schedule: 8am-4pm M-F, with flexibility
