Job Description:

SUMMARY OF RESPONSIBILITIES: The Functional Family Therapy-Therapeutic Family Case Manager (FFT-TCM) will primarily provide case management services to referred families consistent with the evidence-based FFT-TCM Model. Case management services will be delivered weekly or according to the family’s treatment plan, in home-based family settings, to clients in Albany County. This staff will utilize the FFT-TCM model with families who have youth between the ages of 4 and 18. This staff will be required to carry a caseload as assigned, providing services anywhere from 8 to 26 weeks. This position requires flexibility in hours to meet families when they are available.

FUNCTIONS:

* Carry a caseload as assigned

* Provide home-based Functional Family Therapy-TCM to families, honoring model fidelity

* Actively participate as part of an interdisciplinary team

* Perform all case management activities

* Perform the basic accountability tasks related to the job, which include compiling and maintaining all necessary records

* Assist administrative staff in collection of outcome data

* Attend regular supervision and case consultations

* Attend required training

* Read required books

* Responsible for complying with rules and regulations governing access to Protected Health Information under HIPAA. Access to Protected Health Information is limited to the extent required to perform responsibilities effectively and efficiently. No access will be permitted unnecessarily. These essential job functions dictate the level of access, use, and disclosure of confidential information

* Perform other duties as requested by your immediate supervisor, the Senior Director of the region, the VP of CBI, the Chief Operations Officer of CNY, or the Chief Executive Officer

 

Experience and Skills:

QUALIFICATIONS:

* Master’s degree in a Human Services field preferred or Bachelor’s degree, with two years of experience in children and family services

* Excellent communication and organizational skills

* Excellent writing skills

* Ability and willingness to work with families in their own homes

* Flexibility to work when the families are available to meet

* Ability to work independently, as well as contribute to a team

* Strength based perspective

* Attend all provided trainings for the FFT-TCM Model

* Valid NYS Driver’s License

* Vehicle required: Must be safe and dependable

* Cayuga Centers advises that he/she let his/her insurance company know if he/she uses his/her vehicle for agency business

* Mileage, Tolls, & Parking are reimbursed through agency payroll

* Ability to travel to other agency locations as required

 

HOURS PER WEEK: Exempt.

* Regular hours entail a flexible, variable schedule with 40 hours per week

* On-call duties, as assigned
