Job Description:

JOB OBJECTIVE:Under the direction of the Vice President of the CBI Programs and direct supervision of the Director of Cayuga Family Resiliency, the counselor will provide intensive home, school and community based clinical, crisis intervention and case management services to families with children in Cayuga County.


1. Carry a full caseload of 2-3 families assigned.

2. Provide home-based, solution focused case management support and/or crisis interventions to individuals and families both as primary and back-up family worker.

3. Actively participates as part of a team of service providers and others with focus on well coordinated services for family members.

4. Assists individuals and families by actively referring them to appropriate longer-term services and supports.

5. Must be ready, willing and able to assist the families with any and all tasks needed to stabilize them with the intention of modeling and teaching basic life skills to empower individuals and families to become self-sufficient.

6. Performs the basic accountability tasks related to the job which include compiling and maintaining all necessary records, forms and reports.

7. Responsible for maintaining the privacy and confidentiality of Protected Health Information of service recipients in any form (eg., computer access, conversation, document) during and surviving the termination, expiration or cancellation of employment/association with CHC.

8. Responsible for complying with rules and regulations governing access to Protected Health Information under HIPAA. Access to Protected Health Information is limited to the extent required to perform responsibilities effectively and efficiently. No access will be permitted unnecessarily. The essential job functions as described, herein, dictate the level of access, use, and disclosure of confidential information.

9. Provides on-call support as assigned

10. Assists administrative staff in collection of information for program effectiveness.

11. Perform other duties as requested by your direct supervisor or the Chief Executive Officer.

12. Responds in a positive and respectful manner to diversity, among the agency’s service population and staff.

13. Provide crisis interventions defusing problem situations on a 24-hour-day on-call basis.

14. Coordinates and facilitates parent group/Adolescent group

HOURS PER WEEK: Non exempt

Monday-Friday 9:00 am - 5:00 pm with ability to flex hours. Evening work is needed regularly; Weekend work occasionally.


Experience and Skills:


1 Bachelor's in Human Services or related field required.

2. Ability and willingness to work with families in an 'in-home' context.

3. Ability to internalize and work within the general 'Home Builders' philosophy and program model.

4. Knowledge of family systems theory and home based interventions.

5. Ability to engage families who may not want the service or who are mandated.

6. Ability to handle confidential material in a responsible manner.

7. Ability to have flexibility in hours worked both routinely, so as to meet children and families when they are available, and occasionally, in response to crises which may occur at any hour of any day.

8. Vehicle required: Must be safe and dependable.
