Job Description:

SUMMARY OF RESPONSIBILITIES: Coordinates treatment planning and delivery for all program youth and family. Strives to build a partnership between the youth, foster family and biological family. Provides treatment in the forms of individual and family therapy to these assigned youth and families. Maintains professionalism in all areas including confidentiality.


  • Flexible schedule to meet program needs including evenings and weekends as needed; regular hours 9:00am - 5:00pm, Monday through Friday

  • Participate in on-call rotation and availability to respond in person as needed.



  • Complete all required clinical sessions as well as documentation (case notes, treatment plans, mental health assessments, etc.) on time

  • Provide weekly individual therapy and skill building activities to program youth and bi-weekly family therapy to program youth’s family using TFFC treatment model which includes Trauma Systems Therapy

  • Complete biopsychosocial assessments, mental health assessments, Trauma Systems Therapy treatment plans, substance abuse screening assessments, and participates in completion of FASP’s for program youth through the State Connections System

  • Participate in 24 hour on-call availability on a rotating basis

  • Participate in weekly clinical team meetings

  • Provide record keeping and documentation for all clinical activities (case documentation)

  • Utilization of interventions drawn from evidence-based psychotherapeutic methodology

  • Support the youth’s adjustment to foster care

  • Provide training to foster parents as needed

  • Works in collaboration with the psychiatric nurse practitioner and provides psychiatric supports

  • Collaborates with the Health and Wellness Center as needed

  • Coordinate Treatment based services/supports including school, County Case Workers, Probation, Court, CHAD, and home visits, medical care, as well as other ancillary services

  • Respond in a positive and respectful manner to diversity, among the agency’s service population and staff

  • Attend required training

  • Adhere to professional code of ethics (example: NASW Code of Ethics)

  • Responsible for complying with rules and regulations governing access to Protected Health Information under HIPAA. Access to Protected Health Information is limited to the extent required to perform responsibilities effectively and efficiently. No access will be permitted unnecessarily These essential job functions dictate the level of access, use, and disclosure of confidential information

  • Perform other duties as requested by your immediate supervisor or the Chief Executive Officer

Experience and Skills:



  • Master’s Degree in social work or related field. LCSW, LMHC, LMSW - Licensure required

  • Possess the knowledge, attitude, and skills needed to appreciate and encourage diversity

  • Adhere to agency competencies

  • Experience using a trauma informed treatment model

  • Experience providing home-based services preferred

  • Experience treating families, children, and adolescents

  • Ability to complete clinical case documentation and treatment planning

  • Experience completing comprehensive mental health assessments and treatment plans

  • Valid NYS Driver’s License

  • Vehicle required: Must be safe and dependable. State law mandates that one’s personal automobile insurance is directly responsible in any vehicle accident. Cayuga Centers vehicle insurance can legally only supplement damages collected by the other party which exceeds one’s insurance coverage

    • Cayuga Centers advises that he/she let his/her insurance company know if he/she uses his/her vehicle for agency business.

    • Mileage, tolls, & parking are reimbursed through agency payroll
