Job Description:

JOB OBJECTIVE: The care manager coordinator oversees the activities of care managers to assure the provision of high-quality, family-driven, youth-guided service that is in accord with the policies and regulations of the agency, the Health Home and NYS Department of Health. The care manager coordinator will support care managers in their efforts to help families and children advance the health care status of youth eligible for Health Home service.


  • Observe agency personnel policies and assure compliance by supervisees
  • Review documentation by supervised care managers, confirming regulatory adequacy and timeliness
  • Assure that care management activity is consistent with the principles, regulations and policies associated with Health Home Care Management
  • Provide consultation to care managers concerning outreach, enrollment, the development of Plans of Care and the conduct to care managers continuing care management activity
  • Provide supervision to care managers
  • Collaborate and attend required trainings for multiple Health Homes
  • Utilize the analysis of Quality Improvement data.
  • Evaluate impact of care managers’ activity on the health care status of clients, using QI data and observation.

Experience and Skills:


  • Master’s degree in social work or related field, with a minimum of one year experience strongly preferred
  • Bachelor’s degree in human services related field, with a minimum of three years’ experience
  • Fluent with the concepts of family-driven, youth-guided care, the concept of family as natural care manager and understands that improvement in the health care status of the youth is the single most important metric
  • Ability to maintain a detailed awareness about the development and execution of multiple Plans of Care
  • Ability to communicate effectively with a diverse caseload of supervisees
  • Excellent written and verbal skills
