Job Description:

JOB OBJECTIVE:Provide family support intervention to Albany County families.


  • Carry a full caseload of 11 families as assigned.
  • Provide home-based, trauma focused case management support and/or crisis interventions to individuals and families both as primary and back-up support.
  • Actively participate as part of a team of service providers and others with focus on well-coordinated services for family members.
  • Assist individuals and families by actively referring them to appropriate long-term services and support.
  • Assist the families with any and all tasks needed to stabilize them with the intention of modeling and teaching basic life skills to empower individuals and families to become self-sufficient.
  • Compile and maintain all necessary records, forms and reports.
  • Attend required training.
  • Read required books.
  • Responsible for complying with rules and regulations governing access to Protected Health Information under HIPAA. Access protected Health Information is limited to the extent required to perform responsibilities herein, dictate the level of access, use, and disclosure of confidential information.
  • Perform other duties as requested by your immediate supervisor or the Chief Executive Officer.

Experience and Skills:

  • Master’s degree in social work, counseling, education or other human services profession required.
  • Knowledge of family systems theory and home-based interventions required.
  • Experience in both home-based and out-of-home family and children’s services preferred.
  • Ability and willingness to work with families in an "in-home" context.
  • Ability to internalize and work within the general "Home Builders" philosophy and program model.
  • Ability to engage families who may not want the service or who are mandated.
  • NYS Driver’s License required
  • Vehicle required: Must be safe and dependable
