SUMMARY OF RESPONSIBILITIES: Provides independent Medicaid Service Coordination consistent with agency/OPWDD philosophy to individuals with developmental disabilities. Assists families and individuals in assessment, linkage & referral to supports & services, monitoring for satisfaction and service delivery, advocacy, follow-up as needed and maintenance of documentation in case record.


  • Provide linkage and referral with other service providers as needed.
  • Assist consumers to identify goals and documenting these goals.
  • Complete a written individualized service plan for each participant by deadlines set by sponsoring agency.
  • Review the individualized service plan semi-annually, or at the request of the consumer and, as a requirement of OPWDD, rewrite annually.
  • Monitor and coordinate the delivery of services, ensuring maximum benefits/entitlements.
  • Maintain a minimum of one (1) face-to-face contact with the participant in each calendar month.
  • Responsible for maintaining the privacy and confidentiality of Protected Health Information of service recipients in any form (ie: computer access, conversation, document) during and surviving the termination, expiration or cancellation of employment/association with Cayuga Centers.
  • Responsible for complying with rules and regulations governing access to Protected Health Information under HIPPA. Access to Protected Health Information is limited to the extent required to perform responsibilities herein, dictate the level of access, use, and disclosure of confidential information.
  • Responds in a positive and respectful manner to diversity, among the agency’s service population and staff.
  • Perform other duties as requested by your immediate supervisor or the Chief Executive Officer.


Experience and Skills:

  • Bachelor’s Degree or higher in related field with 20 hours in Health and Human Services required
  • One yearof experience working with people with developmental disabilities or one year case management experience with any population.
  • Possess the knowledge, attitude and skills needed to appreciate and encourage diversity including:
  • The ability to forge a mutually respectful partnership with persons served and their families in which they are helped to gain the skills and confidence to address any issues and problems they face;
  • A strong positive conviction about the capacity of people to grow and change;
  • The ability to work in a partnership with other team members;
  • The ability to set limits and maintain the helping role of the practitioner and to intervene appropriately to meet the needs to the persons served or other family members.
  • Valid NYS Driver’s License.
  • Vehicle required: Must be safe and dependable.
  • Bi-lingual-English-Spanish preferred

Schedule:Monday -Regular hours entail Flexible schedule with ability to work from home.
