Job Description:

SUMMARY OF RESPONSIBILITIES: Coordinates treatment planning and delivery for all program youth and family. Strives to build a partnership between the youth, foster family and biological family. Provides treatment in the forms of individual and family therapy to these assigned youth and families. Maintains professionalism in all areas including confidentiality.


  • Provides individual therapy to program youth and/family therapy to program youth’s family using TFFC treatment model which includes Trauma Systems Therapy
  • Completes psychosocial assessments, mental health assessments, Trauma Systems Therapy treatment plans, and participates in completion of FASP’s for program youth through the State Connections System.
  • Participates in 24 hour on-call availability for program youth and parents
  • Attends weekly clinical team meetings
  • Provides record keeping and documentation for all clinical activities (case documentation)
  • Support the youth’s adjustment to foster care.
  • Works in collaboration with the psychiatric nurse practitioner.
  • Coordinate Treatment based services/supports including school, County Case Workers, Probation, Court, CHAD, and home visits, medical care, as well as other ancillary services.
  • Part of the on call rotation available to foster parents, and family/discharge resources.
  • Responds in a positive and respectful manner to diversity, among the agency’s service population and staff.
    • Attend required training.
    • Read required books.
  • Responsible for complying with rules and regulations governing access to Protected Health Information under HIPAA. Access to Protected Health Information is limited to the extent required to perform responsibilities effectively and efficiently. No access will be permitted unnecessarily. These essential job functions dictate the level of access, use, and disclosure of confidential information.
  • Perform other duties as requested by your immediate supervisor or the Chief Executive Officer.

Experience and Skills:

  • Master’s Degree in social work or related field. LMSW, LMHC preferred or eligible for Licensure.
  • Possess the knowledge, attitude, and skills needed to appreciate and encourage diversity including:
  • The ability to forge a mutually respectful partnership with persons served and their families in which they are helped to gain the skills and confidence to address any issues and problems they face;
  • A strong positive conviction about the capacity of people to grow and change;
  • The ability to work in partnership with other team members;
  • The ability to set limits and maintain the helping role of the practitioner and to intervene appropriately to meet the needs of the persons served or other family members
  • Experience with a strength based, cognitive behavioral, trauma sensitive approach to treating troubled youth preferred.
  • Experience with intensive in-home service model preferred.
  • Experience treating families, children, and adolescents
  • Experience with the administrative components of treatment (case documentation) preferred
  • Experience with completing comprehensive mental health assessments and treatment plans.
  • Experience or familiarity with Connections documentation a plus.
  • Experience with completing comprehensive mental health assessments and treatment plans.
  • Valid NYS Driver’s License
  • Must meet any required state certification or licensing criteria.
  • Ability to handle confidential material in a responsible manner.
  • Vehicle required: Must be safe and dependable. State law mandates that one’s personal automobile insurance is directly responsible in any vehicle accident. Cayuga Centers vehicle insurance can legally only supplement damages collected by the other party which exceeds one’s insurance coverage.
  • Cayuga Centers advises that he/she let his/her insurance company know if he/she uses his/her vehicle for agency business.
  • Mileage, Tolls, & Parking are reimbursed through agency payroll.
