Job Description:

SUMMARY OF RESPONSIBILITIES:To follow up with discharged youth after release from program to assure continued follow through with case plan and goals for youth.


  • Develop a discharge plan for youth and families to follow
  • Complete discharge evaluations
  • Continued assessment of youth and family dynamic
  • Assist with coordination of additional services
  • Maintain open communication with youth and families to identify at-risk behaviors and early signs of disruption
  • Attend mandated home-visits with families
  • Work collaboratively and communicates clearly and respectfully with colleagues, families, and other service providers.
    • Must handle confidential material in a responsible manner
    • Participate in staff meetings, seminar and training: maintains and applies a current knowledge of internal and external regulations and policies to applicable foster care
  • Attend weekly supervision meetings to report on cases and present for discussion any problems, difficulties or unresolved issues
  • Attend required trainings
  • Responsible for complying with rules and regulations governing access to Protected Health Information under HIPAA. Access to Protected Health Information is limited to the extent required to perform responsibilities effectively and efficiently. No access will be permitted unnecessarily. The essential job functions as described, herein, dictate the level of access, use, and disclosure of confidential information.
  • Performs other related duties as requested by immediate supervisor or the Chief Executive Officer.

Experience and Skills:

  • Bachelor’s degree in social work, counseling, education or other human services profession preferred. Associate’s degree required.
  • Familiarity with ACS, OCFS, OMRDD, OMH, and other relevant municipal and state agencies
  • Ability to work effectively in a team context
  • Demonstrated ability to perform duties with minimal supervision and to exercise greater judgment
  • Ability to travel to other agency locations and youth homes as required
  • Valid NYS driver’s license preferred
  • NYC ACS or private agency child welfare experience required
  • Strong Microsoft Office skills
  • Ability to work well with people from many cultures including recent immigrants, non-English speakers, and LGBTQ children.

Schedule: Exempt. Must be flexible to meet program needs evenings and weekends required. On-call required.
