Job Description:

SUMMARY OF RESPONSIBILITIES:To coordinate intake/admissions process for each youth that enters our program. The Intake Worker will coordinate the initial program orientation, preparation of materials & supervision of youths and documentation needed during the Intake Admissions Process.


  • Review & confirm names of youth entering program and admit them as they arrive to our facility
  • Confirm the number and names of youths expected in admissions daily
  • Perform initial intake assessment and interpret results providing recommendations and information to all departments regarding any important information provided by minor.
  • Evaluate any immediate safety concerns for minor and/or agency
  • Upon minor admissions:
    • Assure all Property Inventory is collected, documented & place in safe
    • Assure all paperwork is collected upon admission
    • Collect and confirm all Foster Parents Placements with the Homefinding department
  • Facilitate Welcome & Intake Orientation and conduct a mini tour of site
  • Assemble Placement Authorization and Medical Authorization Form information and ensure brochures are complete with minor demographics and provided to Foster parents
  • Serve as liaison for home finding for foster parents during Intake Admissions providing them with documents and mini reminder orientation of their responsibilities
  • Provide daily inventory count of clothing and supply usage during intake
  • Report intake completion daily
  • Attend required training
  • Responsible for maintaining the privacy and confidentiality of Protected Health Information of service recipients in any form (e.g., computer access, conversation, document) during and surviving the termination, expiration or cancellation of employment/association with CC.
  • Responsible for complying with rules and regulations governing access to Protected Health Information under HIPAA. Access to Protected Health Information is limited to the extent required to perform responsibilities effectively and efficiently. No access will be permitted unnecessarily. The essential job functions as described, herein, dictate the level of access, use, and disclosure of confidential information
  • Perform other duties as requested by your immediate supervisor or the Chief Executive Officer
  • Responds in a positive and respectful manner to diversity, among the agency’s service population and staff

Experience and Skills:

  • Bachelor’s Degree in Psychology, Social Work or Counseling required
  • Required experience working in a busy human services office
  • Strong communication, organizational skills and ability to handle numerous tasks required
  • Strong skills in Microsoft Office, Word, Excel and Power Point
  • Strong data entry skills
  • Strong knowledge of clerical and office procedures
  • Bilingual- English / Spanish preferred
  • Good knowledge of English, grammar, spelling, and punctuation
  • Good proofreading and editing skills
  • Ability to handle confidential material in a responsible manner
  • Ability to work effectively in a team context
  • Demonstrated ability to perform duties with minimal supervision and to exercise greater judgment
  • Competency to make imperative decisions
  • Ability to travel to other agency locations as required
  • Possess the knowledge, attitude, and skills needed to appreciate and encourage diversity
  • Valid NYS driver’s license required

Schedule: Full time, hours vary. Must be flexible to meet program needs.
